46 Yr female 

Came with chief complaints of fever with chills since past 3 days, and vomiting since past 4 days.
Patient was apparently asymptomatic 5 days ago then she developed fever which was low grade not associated with chills and not associated with diurnal variation in temperature. She also complained of decreased appetite and generalized body pains associated with fever.
There were no complaints of hematuria, melena or other bleeding manifestations.
There were no complaints of chest pain,
SOB, syncopal attacks.

Not a known case of DM, HTN, CAD, epilepsy
H/o of vitiligo since 3 yrs and uses some ointment.

S: fever,generalised body pains,headache,decreased appetite since 5days.

O: pt is conscious, coherent,cooperative.
BP: 120/80mmhg

GRBS: 190mg/dl - 8am
Spo2: 98%
RS: BAE+ clear
CVS: s1,s2 no added sounds
P/A: not tender
CNS:NAD

A: DENGUE WITH THROMBOCYTOPENIA

P:
1. IV fluids 1 NS 1 RL @75ml/hr
2. Inj.Neomol 1gm/iv/sos (if temp>102f)
3. Inj. Optineuron 1amp in 100ml NS/iv/OD
4. Temp charting 4th hrly 
5. Monitor vitals inform SOS
